Eligibility Information
How do you know if you're eligible for services?
Before you can receive help from Carroll County Board of Developmental Disabilities, the person wanting services needs to be found eligible for services. Being eligible means the person meets a checklist of requirements that defines eligibility. The initial request for eligibility can take place at any age, but eligibility is required to be re-determined at ages 3, 6, and 16. The CCBDD Intake and eligibility process follows applicable Ohio Revised Code for process, timelines, and procedures.
People requesting eligibility must have:
Proof of residency in Carroll County
Birth certificate or other proof of age
Evaluations showing the disability occurred before age 22
Ages 0 to 3
Early Intervention Services are provided to families of children up to the age of three who exhibit delays in development or who may be at risk for delays in development due to medical complications or environmental factors. Ages 3 to 5
Each school district determines whether a student is eligible for admission into the district's pre-school program. If at least two delays are noted in the Evaluation Team Report (ETR) completed by the child's school district, or through a medical evaluation, the child is also eligible for CCBDD services. Ages 6 to 15
A child who is between 6 and 15 years old is eligible for our services if he or she meets the following criteria:
- Has a diagnosis of a developmental disability. A qualified professional must make the diagnosis. A diagnosis verification form must be submitted verifying the diagnosis.
- Has a diagnosis that is expected to be a lifelong disability.
- Has a parent or legal guardian who must give permission for the eligibility determination instrument to be completed with the child.
- Has a substantial functional limitation in at least three of the following areas of major life activity, as appropriate for one's age: self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living.
Ages 16 and over
To be determined eligible for services in this age group, the person must meet the following criteria:
- Have a diagnosis of a developmental disability. The diagnosis must be made by a qualified professional. A diagnosis verification form must be submitted verifying the diagnosis.
- Have a diagnosis that is expected to be a lifelong disability.
- The onset of the disability must have occurred prior to the age of 22.
- The individual or legal guardian (parent if under age 18) must give permission to complete the OEDI.
- Has a substantial functional limitation in at least three of the following areas of major life activity, as appropriate for one's age: self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, and capacity for economic self-sufficiency.

Eligibility is based on meeting certain criteria laid out by the State of Ohio.

Who needs to call?
The individual or the legal guardian of the individual must call in. Our services are voluntary and we need to know that you are aware of the process and that you wish to proceed with the process.
What do I need to do next?
The intake Specialist will provide you with a Diagnosis Verification form and a list of other documents you will need to gather and return to the Intake Specialist.
The list consist of the following:
- A copy of the individual’s birth certificate
- A copy of the individual’s Social Security card
- A copy of the individual’s qualifying diagnosis
- Any school plans, such as a recent IEP and ETR

What happens after CCBDD receives my form and supporting documents?
Once we receive all your documents, a case will be opened for the individual and you will contact you to set up a time to complete an assessment. The assessment allows the Eligibility Team to better determine the individual’s capabilities and whether there is a need for County Board Services.
Once the assessment is complete, you will receive a letter regarding the results of the determination.
- If found Eligible, you will be directed to who to call to determine and receive services.
- If found NOT Eligible, you will be directed to other resources that may be available to you. If you feel that the determination is incorrect, information regarding the appeals process will be included.
